Replace My Membership Certificate

For their protection, members are asked to return their original membership certificate to the office upon change of beneficiary, taking a refund, or completing paperwork when the death payment is issued. If the original certificate has been lost or destroyed, please complete and return the "Lost Certificate Affidavit" which can be downloaded below.
